The worthy winner, along with three friends, will enjoy a pre-match meal along with luxury seats in the Eze Group Stand when Blues take on Reading on Tuesday 31 January.
In December, Eze Group launched the Captain’s Club initiative – designed to recognise and reward captains in the community and up and coming captains of industry.
Eze Group CEO Dominic O’Reilly said: “I think some people overlook how important captains are, not just in sport but in everyday life. A good captain will lead by example and motivate individuals to succeed.
“We’re looking to reward captains from all walks of life, whether it be captains of sport, school captains or club captains. We look forward to reading all of the nominations.”

The winner will be able to bring along three friends or family and if under the age of 16 must be accompanied by an adult. Closing date for nominations is 6pm on 24 January.
In December, Andrew Gallon (pictured above) from Solihull won matchday hospitality at the Blues versus Brighton game. He was rewarded because of his exceptional leadership show when captaining Phoenix XXXL – a football team where you have to have a BMI of over 30 to take part in. Andrew and his brother went on to lose a staggering 11 stone between them.
